- Wellness Booklet can help you get familiar with the different domains of wellness and why they are important.
- The Care Map is a way to create a picture of all the people involved in your care, from your parents to your doctors and people in your community at home. Even your friends and teachers are part of the picture. Your friends, for example, provide social support and comfort, an important part of wellness. Parents, caregivers and siblings are free to participate! Share your map with the Wellness Guide.
- The Vision Map will help you get thoughts down on paper about where you are now, where you want to be, and what can help you get to where you want to be. For example, maybe you want to play with your friends without getting tired, or be happier, or feel less afraid about things. All of these are real things—among many other things—that you can get help with through the Chronic Illness Center. The vision map is like a compass. It will help direct the Wellness Guide to your starting point.
