- The Care Map is a way to create a picture of all the people involved in your care and in your life, from your parents to your doctors and people in your community at home. We've added some suggestions, but feel free to add more and create more boxes as you see fit. Parents, caregivers and siblings are free to participate!
- The Vision Map is another tool where you can write down how you currently feel, what you are interested in working on, and what you believe will help ou acheive those goals. The vision map is like a compass —it will help us get to know you so that we can implement a Wellness journey plan together.
