Connect, Build Resilience, and Find Support
The Resiliency Group Medical Visits support young people ages 16 and older living with chronic health conditions. The group offers a welcoming space to connect with peers who share similar experiences, build coping skills, strengthen resilience, and create a sense of community and belonging.
What to Expect
- Group discussions and check-ins
- Educational and interactive activities
- Strategies for coping with the challenges of chronic illness
- Opportunities to connect with and learn from peers
- Individual medical visit with the Nurse Practitioner
- During each session, participants will take turns stepping out of the group for a brief, 10-minute individual medical visit with the Nurse Practitioner.
- After the visit, participants will rejoin the group.
Led by the Wellness Center’s Nurse Practitioner and Social Worker, these activities are designed to support participants as they build resilience and navigate life with a chronic health condition.
