Clinical Care Design & Delivery:

The Clinical Care Design & Deliver workgroup has been working with the UCSF Clinical Innovation Center design consultant Jan Yeager to develop  the concepts and principles surrounding the Chronic Illness Center wellness programs and the clinical space. We are developing a prototype wellness program supported by  personalized tools, which will enable patients and providers to provide more comprehensive wellness care.
